If you've ever asked a marketing agency about reputation, the answer was probably some version of: "get more Google reviews."
It's standard advice. It's not wrong. It's also no longer sufficient — and for AI visibility specifically, it's a much smaller piece of the puzzle than the volume of advice on the subject suggests.
Here's what AI engines actually weigh when they're forming an opinion about your clinic, and where reviews fit.
What Google reviews actually do
Google reviews influence three things:
- Local pack ranking — your visibility in the map results when someone searches for clinics in your area
- Click-through rate — patients are more likely to click a result with five stars than four
- Last-mile conviction — many patients check reviews as the final step before booking
These all matter. None of them are obsolete. But all three are about getting clicked, getting trusted at the booking stage, or getting found locally on Google. They aren't, for the most part, what AI engines are reading when they decide whether to mention you in an answer.
What AI actually weighs
When AI describes your clinic, it's drawing on a much wider mix of signals. Reviews are part of it, but so are:
- Press coverage, especially named bylines and substantive interviews
- Industry directory entries, especially professional bodies (BAAPS, BACN, JCCP, ACE, IAAFA)
- Podcast and editorial appearances by your clinical lead
- Reddit, forum, and community discussion about your clinic specifically
- Your own site's content, particularly named-practitioner pages and substantive editorial
- The shape of your reviews, not just the number — specific phrases, repeated themes, descriptive language
That last point is the one most clinics miss.
Why review content matters more than review count
Two clinics:
- Clinic A has 500 reviews, average 4.8 stars, mostly one-line "great team!" reviews
- Clinic B has 180 reviews, average 4.7 stars, with patients regularly mentioning "natural-looking results" and "Dr Khan's careful approach"
Clinic A has the better Google review profile by every traditional metric. Clinic B has the better AI visibility profile by a meaningful margin.
The reason: AI engines parse content. The recurring phrase "natural-looking results" associated with Clinic B becomes part of how the model represents that clinic. Clinic A's 500 generic reviews give the model nothing specific to attach.
This isn't theoretical. We've seen clinics with smaller review counts but richer review content come up in AI answers more often than competitors with twice the reviews.
What this means for your review strategy
A few practical shifts:
Stop chasing pure volume. Past a certain point — usually around 50-100 quality reviews — additional volume has diminishing returns. The next 100 generic reviews are worth less than 20 specific, thoughtful ones.
Encourage detail. Train your team to ask patients the right way: "If you have a moment to leave a review, the most useful thing for other patients is to mention specifically what you came in for and how you felt about the result." This produces reviews with content AI can actually parse.
Don't over-engineer it. AI engines (and Google's review filters) are increasingly good at detecting reviews that look templated or coordinated. A real review with a typo and a specific anecdote is worth ten polished ones that all sound the same.
**Look at your review language.** Read your last 30 reviews. What words come up repeatedly? "Friendly," "professional," "clean" tell AI nothing distinctive. "Subtle," "no downtime," "didn't oversell," "explained everything" — these are the phrases that build a positioning.
What else moves the needle
If you've been pouring your reputation budget into Google reviews and seeing diminishing returns, here's where else that energy could go:
Treatment-specific reviews on niche platforms. RealSelf, Trustpilot, and procedure-specific forums often carry more weight in AI training data than another generic Google review.
Practitioner-level reviews. A patient who reviews "Dr Khan" by name builds a personal brand asset that compounds. AI engines increasingly treat practitioners as entities in their own right, separate from the clinics they work in.
Forum and community presence. A clinic whose name shows up in Reddit threads, in Facebook group recommendations, or in patient communities often outperforms one with a much larger formal review base. These mentions are messy, untrackable, and hard to engineer — which is exactly why they carry weight.
Press, podcast, and editorial coverage. Worth more, signal-wise, than several months of review accumulation. A single interview with your clinical lead in a substantive publication can shift how AI represents you in ways no number of Google reviews will.
The honest summary
Google reviews are a hygiene factor. You need a baseline. Beyond that baseline, they're not where the next leg of your reputation growth comes from — at least not for AI visibility.
The next leg comes from being talked about substantively, in places that matter, by people whose voices the models trust.
